  • Submitting Payment

    Payments are submitted at the beginning of each session. Your therapist will ask for your card during your initial session and will be saved and run for subsequent sessions. If you need to change the card on file, you will need to let your therapist know at the beginning of your session.

  • Cancellation Policy

    We understand that sometimes illnesses, emergencies, and more may require you to cancel a session. We ask that you notify us at least 24 hours in advance of your scheduled appointment. Appointments that are not cancelled within 24 hours of the scheduled appointment time and appointments that you are late by more than 15 minutes will be billed at your provider’s full self-pay rate.

    If you repeatedly miss appointments or we are unable to reach you for a period of time, you may be discharged and provided referrals to other providers.

  • Emotional Support Animals

    Therapists at Candor Therapy Network, PLLC are happy to discuss your individual needs for an emotional support animal during your session. Each person’s experience and need are different and your therapist will best be able to assess your individual need. please let your therapist know during your appointment if this is something you are interested in discussing.

  • Gender-Affirming Care

    If you need a letter for another medical provider to allow for gender-affirming care, please let us know during your initial consultation. We can connect you with a therapist in our practice that is comfortable and familiar with completing the required documentation.

  • FMLA / Disability Paperwork

    Therapists at Candor Therapy Network, PLLC are happy to complete FMLA and other disability paperwork as appropriate and following at least eight sessions where we can assess clinical need. Please note that the completion of paperwork outside of session hours or on an urgent basis will be assessed a fee of $119. Additionally, the existence of this policy does not guarantee completion of the requested paperwork if it is deemed not clinically necessary.

  • Legal Appearances / Paperwork

    Therapists at Candor Therapy Network, PLLC do not attend court on behalf of clients. If we do receive a subpoena, a contract and deposit of $2,000 will be required in advance of the hearing date. A minimum of $800 per day will be charged from the deposit for the day of appearance and $.60 per mile driven to and from the hearing appearance. Additional fees may be required if necessary with a minimum fee for the entire appearance being $1,500.
